Given that we accept all major insurance plans and are out of network with many, Harbor employees with health insurance will have their coverage verified to determine their financial responsibility (copay/coinsurance/deductible amount).

All Financial Responsibility through insurance over will be capped at a $60 maximum: For instance, if a Harbor employee's co-insurance is $80, it will now be $60.

For All Uncovered or Cash Pay Treatment Sessions: It will be $60 per session after the 5 complimentary sessions.

The 5 complimentary sessions can be paired with insurance plans to maximize the number of sessions for those who need longer or more consistent treatment.

This perk is NOT extended to family members on the plan.

Our App allows PT to assign custom exercises and feedback. This is a good way for patients to continue care during and in between session breaks.

Insurance payment and check will still be signed over to Evolve PT.
