HOW CAN PHYSICAL THERAPY HELP RECOVER FROM MENISCUS REPAIR OR RECONSTRUCTION SURGERY?
Knee injuries are a common condition treated by physical therapists, and meniscus tears are among the most common of knee injuries, affecting as many as 1 million people in the United States each year. While not all meniscus tears require surgery, meniscus repair and reconstruction techniques are commonly used to help restore as much function to the meniscus as possible. Physical therapy is commonly recommended after a meniscus tear to slowly restore function in the knee while ensuring the surgical repair has time to fully heal.
WHAT DOES PHYSICAL THERAPY FOR MENISCUS SURGERY LOOK LIKE?
My team and I will help you recover from meniscus reconstruction or repair surgery. Most surgeons provide a postsurgical protocol that will help guide your therapist on how much strength, range of motion and activity your knee can tolerate at any given time following the surgery. In the beginning your PT will work on reducing swelling in the knee, teaching you how to use an assistive device to modify your weight bearing and initiate return of muscle function around the knee. As recovery progresses you will continue through a series of exercises to increase range of motion in the knee, improve strength and neuromuscular control, regain normal walking mechanics and then progress to sport-specific or work-specific training exercises.
PTs PLAY AN IMPORTANT ROLE IN MENISCUS REPAIR AND RECONSTRUCTION REHABILITATION?
While a conservative approach to treating a meniscus tear can be successful for many people, in those that have a larger tear that is causing pain, locking or buckling, and is not responding to conservative care like physical therapy, a surgical approach may be recommended. There are three main surgical approaches for a meniscus tear:
Arthroscopic Partial Meniscectomy: During this procedure the damaged part of the meniscus is trimmed away and removed. It is commonly used to treat degenerative tears and radial tears but may not have as good of long term outcomes as a meniscus repair and thus is often not recommended as a first line treatment of choice.
Meniscus Repair:This approach is intended to preserve the entire meniscus by repairing the tear. Not every tear or every patient is a good candidate for this type of surgery but it is preferable when possible to retain as much of the meniscus as possible.
Meniscus Reconstruction: This is a more complex procedure used to replace a partially or totally resected meniscus in symptomatic patients who have been unresponsive to conservative management. The main goal is to fill the defect and safely place a functional meniscus that is similar anatomically and structurally to the native meniscus.
Following the surgery PT will be recommended to help ensure healing of the meniscus repair while restoring function to the knee itself so you can get back to your previous activities.
HOW LONG WILL I NEED PHYSICAL THERAPY?
Recovery from a meniscus repair or reconstruction typically takes longer than for a meniscectomy. Recovery typically ranges from six weeks to three months. During that time your PT will focus their care on helping restore full function of the knee and getting you back to your prior level of activity. Following your therapist’s instructions and adhering to your home exercise program is one way in which you can help reduce the timeline for recovery. Those who are looking to get back to a high level of activity, for example athletes, may require a longer recovery time to ensure that the knee is ready to handle the stresses associated with playing and competing in their sport. If you decide to work with a physical therapist to help with meniscal repair surgery rehab issues, your entire treatment plan could consist of around 8-20+ different physical therapy sessions that will each last 60-90 minutes. Once you complete your customized physical therapy treatment plan, you will be able to continue to do the prescribed stretches and exercises utilized during your PT sessions yet in the comfort of your own home.
Looking for the cause of an injury
While full recovery and return to play is the ultimate goal when it comes to injury rehabilitation, a physical therapist has much more to offer. We are interested in identifying the factors that may have predisposed you to the injury in the first place so that we can make a plan to address them. This helps decrease your risk for future injury. Here are some factors that we look for when assessing a player:
- Muscle strength, power and endurance
- Flexibility and mobility in muscles and joints
- Sleep, nutrition and recovery time
- Training schedule
- History of prior injury
- The physical demands of a player’s position on the team
While some injuries are unavoidable, there may be times when internal and external factors play a role. Identifying these factors allows us to address them with a good physical therapy program or give advice on beneficial modifications to your training.
Movement and Performance Analyses
Over many years of studying the game of football, experts have determined the skills and components necessary for optimal performance. Being able to perform at your best requires that your body has enough strength, power, endurance, flexibility, etc. to not only perform your role on the field but also to manage the high training volumes on the field and in the training room. As movement experts, your physical therapist can assess your form with common training exercises like deadlifts, squats, plyometrics or running and give you insight on how to optimize the movements to enhance effectiveness and also decrease injury risk. Similarly, they can analyze common movement patterns utilized during play and determine if there are areas of weakness or impairments that should be addressed through a comprehensive training program.
