Physical Therapy for Strengthening Program

HOW CAN I BENEFIT FROM A STRENGTHENING PROGRAM?

Strengthening is recommended for all adults and many adolescents. In fact, the Centers for Disease Control and Prevention recommends that all adults participate in a total-body strengthening routine two days per week. Strengthening is used to achieve many goals including improving general health, enhancing function, optimizing sport performance and recovering from an injury, disease or illness.

HOW LONG WILL I NEED PHYSICAL THERAPY?

The length of a physical therapy plan of care may last only a few visits if the goal is to establish an exercise program for an uninjured player looking to reduce future injury risk and maximize performance. It could also last much longer if a player needs to undergo rehabilitation for a specific injury or following a surgery. Your therapist will discuss this with you at your first visit so that you know what to expect.

WHAT DOES A STRENGTHENING PROGRAM LOOK LIKE IN PHYSICAL THERAPY?

In order to prescribe a strengthening program your physical therapist will start with a subjective interview. They will ask you about your recent and past history with exercise, what kind of exercise equipment you like using and have access to, your beliefs about exercise and about any injuries, illnesses or surgeries you have recently experienced. Then, your therapist will perform a physical exam wherein they will examine any injuries you have and assess different components of movement that will help them develop an appropriate strengthening program. During the physical exam your therapist may do the following:

  • Perform manual muscle testing to test the strength of different muscles
  • Measure or assess the length of muscles
  • Measure the range of motion around joints
  • Assess the mobility at different joints
  • Palpate tissues to look for pain or tissue dysfunction
  • Have you perform different movements and motions while they analyze your movement
  • Participate in an outcome measure to assess functional strength, endurance, speed or agility
With all this information your therapist can prescribe a strengthening program to help address your goals.

THE PHYSIOLOGY BEHIND STRENGTHENING

When it comes to muscle performance, there are several goals that training can achieve. Each one requires that you train the muscles in a specific way. Failing to train the muscle appropriately can lead to injury and also diminish the success of your hard work. Some of the most common types of muscle performance individuals like to train are:
  • Muscle strength
  • Muscle endurance
  • Muscle power
  • Functional power
  • Neuromuscular control
  • Aerobic fitness
As you can see, muscle strengthening is one of many types of training you may encounter when working with a professional like a physical therapist. Your PT will know which types of training you need to reach your goals. When it comes to building muscle strength, you are aiming for muscle hypertrophy. Building the cross-sectional area of the muscle so that it can generate more force, ie. become stronger. While you will see some variety in exercise prescription for building strength, these are general exercise principles that will help ensure your muscles are growing:
  • Train the desired muscle at least 2x per week
  • Perform 1-3 sets of each exercise
  • Perform 8-12 reps per set
  • Rest 2-3 min between sets
  • Allow at least 48 hours recovery between sessions
  • Perform exercises slow and controlled
  • Aim for an intensity of “somewhat hard to hard”
  • Match exercises with desired function
To build strength, exercises that feel somewhat hard to hard are needed. With this intensity, however, comes the risk for injury if exercises are not performed correctly. Working with a professional who can teach you appropriate exercises, watch your form and give you feedback is important to minimize injuries.

Once you complete your customized physical therapy treatment plan, you will be able to continue to do the prescribed stretches and exercises utilized during your PT sessions yet in the comfort of your own home.

Looking for the cause of an injury

While full recovery and return to play is the ultimate goal when it comes to injury rehabilitation, a physical therapist has much more to offer. We are interested in identifying the factors that may have predisposed you to the injury in the first place so that we can make a plan to address them. This helps decrease your risk for future injury. Here are some factors that we look for when assessing a player:

  • Muscle strength, power and endurance
  • Flexibility and mobility in muscles and joints
  • Sleep, nutrition and recovery time
  • Training schedule
  • History of prior injury
  • The physical demands of a player’s position on the team

While some injuries are unavoidable, there may be times when internal and external factors play a role. Identifying these factors allows us to address them with a good physical therapy program or give advice on beneficial modifications to your training.

Movement and Performance Analyses



Over many years of studying the game of football, experts have determined the skills and components necessary for optimal performance. Being able to perform at your best requires that your body has enough strength, power, endurance, flexibility, etc. to not only perform your role on the field but also to manage the high training volumes on the field and in the training room. As movement experts, your physical therapist can assess your form with common training exercises like deadlifts, squats, plyometrics or running and give you insight on how to optimize the movements to enhance effectiveness and also decrease injury risk. Similarly, they can analyze common movement patterns utilized during play and determine if there are areas of weakness or impairments that should be addressed through a comprehensive training program.
